Senior Care Partners PACE is a full health care provider and the insurance provider for older adults who want to age at home and in their community. Participants with Senior Care Partners PACE have primary care, home care, personal care, transportation for medical appointments, pharmacy/medication, physical therapy, social work and case management, occupational therapy, recreational therapy, registered dietitian, nursing, and more wrapped around them. We are a non-profit and funded by Medicaid, Medicare, and private pay. Over 99% of our participants are eligible for long-term Medicaid. We seek to help aging adults remain independent and active in their communities.
